It was first developed by Dr.Paulo Malo at Malo Clinic, Lisbon, Portugal. All on 4 Implant Procedure was first done in 1993. All on 4 implant procedure is a graft less implant placement procedure to restore edentulous jaws in Upper and Lower jaws by Placing Tall and Tilted Implants. In this procedure, the posterior implants are placed in an angulated or tilted manner for utilizing the maximum amount of bone thereby getting very good bone support and maximum surface area.
Moreover, it allows long implant placement and insertion of the implant apex in high-density bone both in Upper and Lower Jaws.
All on 4 Implant Technique
Four implants are inserted with the back implants being tilted up to 45 degrees. A surgical guide is used for the correct placement of implants. You can do both Flap and Flapless techniques in this All on 4 Technique. To compensate for the 45-degree angulation of the implants placed, we use special components to make the prosthetic components straight; thereby we can restore the implants immediately after the surgery. A cantilever pontic (Extra tooth) can be added to the final prosthesis. The skilled procedure of tilting the posterior implants avoids expensive, time-consuming, and more invasive grafting procedures like sinus grafting, block grafting, etc. Moreover in the conventional technique, we have to wait for at least 3 to 6 months which is not so in this procedure.
